Transitioning from a focus on disease treatment to wellbeing, prevention and safe self-care requires a redesign of social institutions and governance mechanisms and coordination between parties across professional, organizational and sectoral boundaries.

Supporting people and protecting their health also increases the need for informal care and social networks, as well as the importance of expertise in healthy lifestyles. New initiatives often encounter institutional barriers and governance dilemmas between, for instance, decentralisation and centralisation or integration and differentiation. This calls for understanding the paradoxical effects of new policies.
